Summer 2025
First Day of School is August 20!
If your child is planning on participating in ANY sports next year, a CURRENT sports physical, concussion statement and activity agreement forms are required. Student athletes cannot practice or participate in their sport of choice without them. Appointments can be difficult to get last minute - please plan ahead! Physicals must be completed after June 6, 2025 to be valid for the 2025-26 school year. **Only one set of completed forms are required per school year.
